Rates

DOT SAP Rate: Please call me for rate.  Rates are reasonable and affordable.  Video only due to COVID.  Will return to Face to Face when COVID emergency ends.

Counseling Sessions: $90.00    50 minutes       30 minutes sessions  $45.00    Sessions by phone or video only due to COVID.

Insurance

DOT SAP services are not covered by insurance but may be covered on some EAP plans. Please check to see if I am on your EAP plan. Payments for DOT SAP services are accepted through CASH APP and PayPal. Full payment is expected before the services are rendered. Therapeutic services may be covered by your health insurance policy or employee benefits plan. Please check with your policy provider by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible?
• How many sessions does my health insurance cover?
• What is the coverage amount per session?

Payment

Cash and all major credit cards are accepted for payment.  PayPal and CASH App accepted

Cancellation Policy

You must notify us at least 24 hours in advance if you wish to cancel your DOT or therapy appointment. Otherwise, you will bear a charge of $50.00.  Missed appointments charges must be paid before a service is rendered.
